Medical Form Required for Weekend Campouts
BSA Annual Health Form Parts A, B1, and B2 (Updated 9/19/2022)
- Scouts and Adult Leaders are not permitted to participate in outdoor activities until forms have submitted.
- Does not include the medical doctor authorization (Part C)
- Needed for any and all activities less than 72 hours (3 nights or less)
- Same form is required for adults and youth
- Must be renewed yearly
- Must include a photocopy of the Scout or Leader’s insurance card (front and back)
Medical/Etc. Required for Summer Camp/High Adventure
BSA Annual Health Form Part A, B1 and B2 & C (Updated 9/19/2022)
- Adds the 1-page medical doctor authorization (Part C), otherwise is the same
- Needed for Summer Camp and other trips in excess of 72 hours (more than 3 nights)
- Same form is required for adults and youth
- Must be renewed yearly. Annual physical MUST be completed within 365 days of Summer Camp 2026. Please see Administration for additional details.
- Must include a photocopy of the Scout or Leader’s insurance card both front and back.
Supplemental Medical-CRK Specific (Added 5/8/23)
- Required for Summer Camp 2025 at CRK
Outdoor Release Form (Added 5/8/23)

- The form requests a “BSA Registration No.” … if you do not know it leave it blank and we will add it
